Important information for filing your COVID-19 claim

  • You will need this information in order to file a claim:

    • Your company control number. Up to 5-digits, this number can be found on information provided to you by your employer, your STD brochure and/or all disability communications. If you cannot locate your group control number, please contact your employer. You need this number to register online and report your COVID-19 claim.
    • Your last day worked and your first day out due to COVID-19.
    • Your diagnosis of COVID-19 and/or symptoms related to COVID-19.
    • Employee ID or Social Security number.
    • Address and telephone number. We recommend providing your cell phone number so we can send you updates quickly and conveniently via text message.
    • Doctor’s name, phone number, and fax (if available).
    • Any underlying conditions you may have.
    • Expected return to work date, meaning if you know when your physician feels you would be able to return to work.
  • If Prudential administers a state-mandated disability benefit on behalf of your employer, filing a claim for disability with Prudential may also qualify as filing for that state-mandated benefit. In addition, you may be entitled to additional disability benefits through your employer or your state. Please check with your employer for information on whether you are entitled to additional benefits and, if so, how to apply for any such benefits.

    2. Visit the Prudential My Benefits web site
      • Log in using your username and password
      • If you are new to this website, register to set up your username and password using your company control number.
      • Submit your claim by following information available on the page after you login. Be sure to indicate your preferred method of contact—text or email—so that you can receive updates on your claim.
      • You will then be taken through step by step instructions starting with a question regarding if your claim is related to COVID-19.

    • Prudential will review the information you provide, confirm your eligibility with your employer and review any pertinent medical information.
    • You do not need to contact us. We will contact you if we require additional information.
    • You will receive a text message and/or email (depending on the information you provided) when a decision is made on your claim.
    • The quickest way to receive updates on your claim with your permission is via text message and/or email. You indicate your preference on the Prudential My Benefits web site when you file your claim.
    • You can find up to date information on your existing claims by visiting Prudential My Benefits web site. You can also provide any required information through this site

  • To file an Absence & Family Medical Leave claim:

    1. Notify your supervisor.
    2. Visit the Prudential My Benefits web site
      • Log in using your username and password
      • If you are new to this website, register to set up your username and password using your company control number.
      • Submit your claim by clicking “Claims and absence” and then “File a Claim / Report an Absence”.
      • Be sure to indicate your preferred method of contact—text or email—so that you can receive updates on your claim.
