Individual LTC3SM Long-Term Care Insurance*
LTC3 policy issued by The Prudential Insurance Company of America (Prudential) offers all of the key elements of a quality LTC insurance plan, including:

Some of the insurance plan benefits built into Prudential's LTC3 Long-Term Care Insurance include:
- Home Support Services Benefit, which equals a lifetime benefit in multiples of your chosen Facility Daily Benefit Maximum to cover items that may help you remain at home. It provides funds for such things as:
- Assistive devices
- Durable medical equipment not covered by Medicare
- Caregiver training
- Home modifications
- Emergency medical response systems
- Transportation services
- Restoration of benefits
- International coverage benefit
- Optional inflation protection choices (Available at an additional cost)
LTC3 Policy Features and Options
The following are the insurance plan features in Prudential's highly flexible LTC3 policy. The premium is based on the benefits selected. (Plan features and options vary by state.)
Elimination Period. An elimination period is the amount of time you choose to pay for LTC before insurance coverage begins. The longer the elimination period, the lower the premium; the shorter the elimination period, the higher the premium. You may choose from elimination periods of 30, 60, 90, 120, 180, and 365 days.
Lifetime Maximum Benefit. Prudential offers Lifetime Maximum Benefits based on lengths of time equal to 2, 3, 4, 5, 6, and 10 years; unlimited is also available.
Facility Daily Benefit Amount. The Facility Daily Benefit amount is the maximum amount the policy will reimburse for institutional care benefits in a single day. When purchasing an individual policy, you can choose from Facility Daily Benefit amounts ranging from $50 to $500 per day in $10 increments.
Home Care Daily Benefit. The Home Care Daily Benefit is established as a percentage of the Facility Daily Benefit Amount. Prudential offers Home Care Daily Benefit Amounts for home- and community-based care at 50%, 75%, 100%, and 150% of the Facility Daily Benefit Amount. The 150% option can only be selected with a Daily Benefit Amount of $350 or less.
California residents: The Home Care Daily Benefit percentage amount elected cannot result in a Home Care Daily Benefit amount that is less than $50.
Home Care Benefit Payment Options. There are five ways to receive home- and community-based benefits: Daily Benefit, Monthly Benefit,* Cash Alternative, Flexible Cash Benefit Rider,* and the Cash Benefit Rider.*
Inflation Protection Option. You have five choices when it comes to protecting the value of your coverage over time:
- Guaranteed Purchase Option (GPO)*—Increases to coverage are made every three years. Policy benefits will increase by the amount derived from applying 5% annual compounding for three years to your current policy benefits. Proof of good health is not required, and increases are made even if the insured has met the benefit eligibility criteria or is in claim.
- Automatic Simple Inflation Benefit*—Each year, there will be an automatic increase to policy benefits equal to 5% of the original policy benefits.
- Automatic Compound Inflation Benefit, 2X Maximum*—Each year, there will be an automatic increase to policy benefits equal to 5% of the current policy benefits until policy benefits equal twice the original policy benefits.
- Automatic Compound Inflation Benefit, No Maximum*—Each year, there will be an automatic increase to policy benefits equal to 5% of the current policy benefits.
- None: The benefit will remain constant and will not inflate.
Mandated offer of Automatic Compound Inflation Protection-No Maximum must be rejected before other forms of inflation protection are offered. This is only applicable to individual long-term care policies.

Exclusions
This is a general list of exclusions, however, state variations may apply. Please see the Outline of Coverage for a complete listing of exclusions, which apply to your state of residence. Your LTC3 policy is designed to provide benefits for your Qualified Long-Term Care Service needs. Benefits will NOT be available if any of the following situations apply.
- Illness, treatment or medical conditions arising out of (a) war or an act of war, whether declared or undeclared, while you are insured*; or (b) your participation in a felony, riot, or insurrection; or (c) alcoholism and drug addiction.**
- Treatment provided in a government facility, unless payment of the charge is required by law or services provided by any law or government plan under which you are covered. This does not apply to a state plan under Medicaid or to any law or plan when, by law, its benefits are excess to those of any private insurance program or other non-governmental program.***
- Charges for services or supplies for which no charge would be made in the absence of insurance.
- Charges for care or treatment provided outside the United States except as described in the International Coverage benefit.
- Charges for expenses reimbursable under Medicare or for expenses that would be reimbursable under Medicare but for the application of a deductible or coinsurance amount.
- Benefits under your Policy may be reduced if Prudential also pays benefits for Eligible Charges under any other Prudential Long-Term Care Insurance Policy. ****
* In Oklahoma, war or an act of war while serving in the military service or any auxiliary unit attached thereto.
** In Louisiana and Vermont, benefits would not be payable for treatment of alcoholism and drug addiction.
*** In North Carolina, treatment provided in a government facility (unless otherwise required by law). Services for which benefits are available under Medicare (unless otherwise required by law), or under any other governmental program (except Medicaid). Services or supplies for the treatment of an Occupational Injury or Sickness which are paid under the North Carolina Workers' Compensation Act only to the extent such services or supplies are the liability of the employee, employer or worker's compensation insurance carrier according to a final adjudication under North Carolina Workers' Compensation Act or an order of the North Carolina Industrial Commission approving a settlement agreement under the North Carolina Workers' Compensation Act.
