As a Verizon employee or retiree, most employees receive Basic Term Life Insurance provided by the company. In addition, you may also have the option of purchasing additional supplemental coverage for you, your spouse, or your children.

In certain situations, you may need to answer medical questions about your health, or Evidence of Insurability (EOI). EOI may be required if you or your dependent spouse enrolls for Life Insurance over the guaranteed issue amount or enrolls in coverage outside of the initial eligibility time frame.
